In Guitar Hero III is het net als in de vorige delen de bedoeling om met de speciale Guitar Hero-gitaar ritmisch mee te pingelen met verschillende rocknummers. Op de maat van de muziek verschijnen bepaalde symbolen in beeld. Deze symbolen zijn gekleurd en elke kleur staat voor een van de vijf toetsen op de gitaar. Door de knoppen op het juiste moment in te drukken in combinatie met een "hoofdknop" kun je spelen als een echte rockgod. In het begin valt dit nog mee, maar op een gegeven moment speel je de meest ingewikkelde gitaarsolo's. De game bevat meerdere moeilijkheidsgraden, en de mogelijkheid te duelleren tegen legendarische gitaristen als Slash.
